We collect certain data from you to meet mandatory requirements regarding medical notes, which we hold electronically, this is password protected. There is a legal requirement to keep medical notes for a minimum time after treatment. This can vary depending on your age and ability to consent but will be for a minimum of 8 years. Your records will be destroyed after this period.
There may be occasions where we want to share information with your general practitioner or medical staff in the event of an emergency.
We also collect data on an excel spreadsheet to assist in the administration of our business to provide you with efficient service. We would like to use your contact details to assist with the administration of your appointments/ changes to scheduled appointments and/ or reminders about appointments. To further enhance our service to you, we would like to be able to update you on any information regarding the practice.
We take your privacy seriously and will take all reasonable steps to ensure the protection of your data. Your information will not be passed over to third parties and is solely used for the purpose for which it was collected. If I sell or merge my business, I may disclose your information as part of that transaction, only to the extent permitted by law and with your consent.
You have the rights to change, restrict the use of or delete your personal information. In the case of health records, these are normally exempt from change and deletion requests
You will be required to sign a data consent form acknowledging your acceptance of this information

Missed appointments or cancellations with less than 24 hours notice will incur a £30 charge.
